You may find the forms below useful for planning:

  • The Retirement Plan Design Questionnaire will provide us the information
    necessary to make recommendations regarding plan design and to complete plan documents.
  • The Census Data Submission Form is a data template requesting information about your company’s
    employees that will allow us to prepare a detailed analysis illustrating the benefits and costs
    of various plan design options.
  • The Takeover Documents Checklist is useful if an existing retirement plan is already in place.
    It lists the various items we will need in order to transition the administration of your existing
    plan to PPPC.
  • The 401(k) Plan Survey will help you determine your employees’ interest in a 401(k) Plan and
    is helpful in designing the provisions of the plan.
  • The Financial Data Summary may be used to summarize your plan’s annual financial activity.
